Responsibilities
- Support psychiatric hospital services, including 5150/involuntary holds, intake and admission screenings, biopsychosocial assessments, and patient and family education, support, and advocacy.
- Work with a multidisciplinary team to promote continuity of care.
- Apply knowledge of adoption and foster care, drug and alcohol use and treatment options, suicide, homicide, and self-harm.
- Apply knowledge of adult protective services, child protective services, emergency detention, guardianship, and power of attorney.
- Assess response to treatment, collect and analyze patient information, conduct biopsychosocial assessments, and complete initial evaluations.
- Provide case management, utilization review support, and care coordination.
- Conduct crisis assessments and mental health assessments, including crisis intervention for teen suicides, drug- and alcohol-related issues, and school violence.
- Provide crisis intervention and management, develop clinical assessment reports, apply ethical principles, and identify social, emotional, behavioral, and trauma-related needs.
- Coordinate with internal and external team members, document pertinent information to facilitate care coordination, and refer patients to other disciplines and resources.
- Apply knowledge of dementia and psychiatric disorders.
- Educate patients and caregivers to facilitate discharge and reinforce individualized patient and caregiver education.
- Support emergency department and psychiatric services in an acute hospital setting.
- Facilitate discharge and contingency planning and provide crisis intervention.
- Apply knowledge of cultural issues and community programs involving Medicaid and Medicare.
Requirements
- Current LCSW, LMFT, or LMHC certification is required.
- Experience with complete intake and admission screenings is required.
- Patient and family education, support, and advocacy experience is required.
- Adoption and foster care knowledge is required.
- Adult protective services knowledge is required.
- Child protective services knowledge is required.
- Power of attorney knowledge is required.
- Care coordination experience is required.
