Responsibilities
- Coordinate patient care across the healthcare team and support effective transitions of care.
- Assess patient needs, support discharge planning, and help arrange appropriate services and equipment.
- Review clinical and administrative information to promote appropriate utilization of care.
- Document case management activities accurately in the electronic health record.
Requirements
- Epic charting experience.
- Experience with admission criteria, appeals and denials, benefits eligibility, care coordination, discharge planning, needs assessment and ordering DME, and prior authorizations.
- Worker’s compensation case management experience.
- Knowledge of CMS (Centers for Medicare and Medicaid Services), DRGs (Diagnosis Related Groups), HIPAA guidelines, The Joint Commission, Core Measures, and National Safety Goals.
- Knowledge of Workers Compensation requirements.
- Acute hospital experience involving the Emergency Department, ICU, MS, Pediatrics, and SDU/PCU/IMC/Observation.

Experience
- At least 2 years of acute hospital case management experience performing care coordination and discharge planning.
