Responsibilities
- Coordinate patient care across the acute hospital setting.
- Support admission review, utilization activities, care coordination, and discharge planning.
- Complete needs assessments and help arrange appropriate durable medical equipment.
- Develop and monitor plans of care while supporting timely transitions.
- Collaborate with patients, families, providers, and interdisciplinary care teams.
Requirements
- Experience with admission criteria, benefits eligibility, disease management, and prior authorizations.
- Knowledge of CMS (Centers for Medicare and Medicaid Services), the Department of Health, and DRGs (Diagnosis Related Groups).
- Knowledge of The Joint Commission requirements, Core Measures, and National Safety Goals.
- Acute hospital experience involving the Emergency Department, ICU, medical-surgical (MS), and Surgical Services.
- Completion of the extra required checklist and review of the additional requirements provided in the application materials.

Experience
- At least 2 years of acute care case management and discharge planning experience.
- Seasoned travel nurse experience is required.
