Responsibilities
- Support patients and care teams throughout the hospital stay.
- Promote timely, organized transitions across the continuum of care.
- Collaborate with interdisciplinary teams to support safe and appropriate patient outcomes.
- Document case management activities accurately and maintain compliance with applicable standards.
Requirements
- Experience working in an acute hospital setting.
- Case management and utilization review skills, including admission criteria, benefits eligibility, care coordination, discharge planning, disease management, needs assessment and ordering DME, plan of care, and prior authorizations.
- Knowledge of CMS (Centers for Medicare and Medicaid Services), the Department of Health, DRG (Diagnosis Related Groups), and The Joint Commission, including Core Measure and National Safety Goals requirements.
- Acute hospital experience involving the Emergency Department, ICU, medical-surgical care, and surgical services.

Experience
- Two years of acute care case management or discharge planning experience.
- Experience with a patient ratio of 1:25.
- Seasoned traveler experience is expected for this assignment, though travel experience is not a hard-stop requirement.
