About This Position

Responsibilities
Support coordinated patient care across the acute hospital setting by evaluating needs, collaborating with interdisciplinary teams, helping patients transition safely through the continuum of care, and maintaining accurate documentation. Use clinical judgment, effective communication, and organized follow-through to support appropriate care progression and resource planning.
Requirements
- Epic charting experience
- Acute Hospital work setting experience
- Case management and utilization review skills, including Admission Criteria, Appeals and Denials, Benefits Eligibility, Care Coordination, Discharge Planning, and Needs Assessment/Order DME
- Prior Authorizations required
- Worker’s Compensation case management required
- CMS (Centers for Medicare and Medicaid Services) knowledge
- DRG (Diagnosis Related Groups) knowledge required
- HIPAA guidelines knowledge
- The Joint Commission, Core Measures, and National Safety Goals knowledge
- Workers Compensation knowledge required
- Experience supporting Emergency Department, ICU, MS, Pediatrics, and SDU/PCU/IMC/Obs populations

Experience
