Responsibilities
- Support safe, timely transitions of patients across the continuum of care.
- Collaborate with interdisciplinary teams, patients, families, and external partners.
- Review patient needs, coordinate appropriate services, and maintain accurate documentation.
- Promote efficient resource use while supporting quality and regulatory expectations.
- Communicate updates clearly and help resolve barriers to appropriate care.
Requirements
- Epic charting system experience required.
- Case management and utilization review skills, including admission criteria, appeals and denials, benefits eligibility, care coordination, discharge planning, needs assessment and ordering DME, prior authorizations (required), and workers’ compensation case management (required).
- Acute hospital work setting experience.
- Regulatory knowledge of CMS (Centers for Medicare and Medicaid Services), DRG (Diagnosis Related Groups) (required), HIPAA guidelines (Health Insurance Portability and Accountability Act), and The Joint Commission, Core Measures, and National Safety Goals.
- Workers’ compensation regulatory knowledge (required).
- Acute hospital experience involving the Emergency Department, ICU, MS, Pediatrics, and SDU/PCU/IMC/Observation.

Experience
- At least 2 years of acute hospital case management experience performing care coordination and discharge planning.
