Responsibilities
- Evaluate patient needs and support appropriate care transitions.
- Collaborate with interdisciplinary teams to promote timely, coordinated care.
- Communicate with patients, families, providers, and payers as appropriate.
- Maintain accurate documentation in the designated electronic health record.
- Support efficient hospital workflows and continuity of care.
Requirements
- Epic charting experience required.
- Acute hospital work setting experience.
- Case management and utilization review skills, including admission criteria, appeals and denials, benefits eligibility, care coordination, discharge planning, needs assessment and ordering durable medical equipment (DME).
- Prior authorizations required.
- Worker's compensation case management required.
- Knowledge of CMS (Centers for Medicare and Medicaid Services) requirements.
- DRG (Diagnosis Related Groups) knowledge required.
- HIPAA (Health Insurance Portability and Accountability Act) guidelines knowledge.
- Knowledge of The Joint Commission, Core Measures, and National Patient Safety Goals.
- Workers' compensation knowledge required.
- Acute hospital experience involving the Emergency Department, ICU, MS, Pediatrics, and SDU/PCU/IMC/Observation settings.

Experience
- At least 2 years of experience.
- 2 years of acute hospital case management experience performing care coordination and discharge planning.
